Do any of my colleagues have any suggestions about management of post-capsulectomy contour deformities characterized by lower pole volume loss and tethering of the nipple/areolar complex (in a patient with a sub-muscular complete capsulectomy).   I believe there was some discussion at the meeting this weekend about autologous fat grafting in this context and what are your experiences with addressing the problem with re-augmentation with or without P4HB.

Joseph, Fat grafting can help smooth out contour irregularities, but I'm why there is tethering of the NAC unless she had a previous periareolar incision. I have seen a few of those have deformities that weren't evident with the pressure of the implant prior to capsulectomy and implant removal (you didn't mention whether you replaced the implant or not). These are more complex due to scarring and not always improvable with fat grafting.
