Joseph, Fat grafting can help smooth out contour irregularities, but I'm why there is tethering of the NAC unless she had a previous periareolar incision. I have seen a few of those have deformities that weren't evident with the pressure of the implant prior to capsulectomy and implant removal (you didn't mention whether you replaced the implant or not). These are more complex due to scarring and not always improvable with fat grafting.
